為避免捐贈生殖細胞個案及受術夫妻日後可能因通訊方式改變或者來自不同國家，後續發生失聯或不易聯絡等情事，致使人工生殖機構在未事先取得相關同意書前，無法依人工生殖法進行生殖細胞或胚胎銷毀、轉贈（移）等情事，爰研擬生殖細胞或胚胎銷毀、轉贈（移）同意書範本提供參考使用。
